How To Use the Online Access Request Form

Access to Research areas via the online access must be requested through our on-line request form. Once you submit your request, your Authorizing Supervisor or your Temporary Authorizing Supervisor will receive an e-mail asking them to approve the request, within 30 days. You will receive e-mails giving you the status of your request.

This online access request platform is only to obtain access to Research areas in LKSKI, 193 Yonge and 250 Yonge. If you need access to NON-Research areas (Hospital, 80 Bond, 61 Queen St. East, etc.), please fill out this Access request form, ask your supervisor to sign and submit directly to security in the main floor of Cardinal Carter wing in the main Hospital (Victoria entrance).

Before Requesting Access:

Before requesting access to Research Areas, make sure you have completed the SMH Research Registration process:

1. Registration with HR (for employees) or Registration with the Office of Research Administration (for students, trainees, visitors, volunteers and other non-paid personnel)

2. Obtain ID badge and Access card from IT (4th floor of Shuter wing – main hospital)

Enter your Access Card number or Log-in to RFBMS

The five-digit number is located on the separate thick white card behind your ID badge. This is NOT the same as the number on the back of your ID badge. Your card will not be programmed if you don’t give us the correct number.

Once you enter your Access card number, click “Confirm Identity”

- You will be prompted to enter your SMH e-mail address. A validation code will be sent to that address. The system will prompt you to enter this code in order to validate your identity - Create a new profile if you are a new research staff. If you are a person who already has access (or had access in the past) but the system cannot find your profile, please do not create a new profile. Please contact ResearchFacilities@smh.ca for assistance.

- To save changes click “Save and Continue”

- Click on the “Add New Record” to select that area that you would like
- Hover over area to see the points included in the group
- Click “Save”
- Another area can be added

Renewal:

To Renew your access to Research areas, you must contact the Office of Research Administration first to ensure that all appropriate paperwork has been submitted. Failure to do so could result in your renewal request being denied.

Please contact Cordelia Cooper or Dalbir Singh to submit the proper paperwork. Once you have submitted the paperwork, you can then renew your access.

FAQ

What is the difference between my Supervisor and a Temporary Authorizing Supervisor?

Your Supervisor is the person who you report to.

A temporary Supervisor is a person who will receive the access request e-mail and can authorize your access request. This is most often use when requesting access to specialized areas, such as dedicated lab spaces, the Research Vivarium or Graduate Seating.

 

What if I need access to 250 Yonge?

Access to 250 Yonge is part of Research Miscellaneous access group, which everyone with access automatically gets.

 

How do I get access to 193 Yonge?

Access to 193 Yonge is granted only to tenants (people who have a permanent seat in the building). Access requires a fob, which can be provided by Research Facilities. To request a fob, supervisors should send an email to Research Facilities with the following information: name of person requiring fob, department and area(s) requested.

 

What if I need access to the 3rd floor LKSKI Bridge to the Hospital?

Access to the 3rd floor LKSKI bridge is part of Research Miscellaneous access group, which everyone automatically gets. If you have access to other areas of Research but you cannot access the bridge, please contact security directly at the main hospital (CC wing, main floor, Victoria entrance).

 

What if my card doesn’t work after 48 hours or if I have issues with my access card?

Please follow up security directly at the main hospital (CC wing, main floor, Victoria entrance).

 

What if I cannot find my Authorizing Supervisor on the list?

Please contact Research Facilities if you cannot find your Authorizing Supervisor. Please note that only Scientists, Associate Scientists and Departmental Managers and Directors can be supervisors.

 

What if my request is not approved within 30 days?

If your request is not approved within 30 days, the request will expire. You will have follow up with your supervisor and place a new request accordingly.

 

How do I access to LKSKI bike cage?

You can find the cage in the LKSKI parking garage next to the entry and exit ramp. To ensure the safety of all those using the parking garage, some security measures have been put in place. Most importantly, there will be a requirement to walk your bike up and down the ramp at all times.
In order to gain access, you must be an employee at the LKSKI.
Step 1: Please complete the following bike cage agreement outlining the safety and security requirements in order to have your bike cage access authorized.
Step 2: Once signed, please submit it to the Research Facilities office (LKSKI, 711)

 

Do I need a smh.ca email account to request access through the RFBMS?

Yes. An smh.ca email account is required to request access in order to validate your identity. If you don’t know your email or have questions about your email, please contact Helpdesk at (416) 864-6060 ext. 5751 or helpdesk@smh.ca
